Join this hands-on, interactive workshop to explore how hybrid care pathways (combining in-person and digital healthcare) can improve patient outcomes and build resilient health systems. Through the lens of fictional patient journeys, youāll collaborate in small groups to identify what care can go digital, what canāt, and how to make it all work in your own setting. This workshop, part of the EU-funded RAPIDE project, offers a unique opportunity to co-create future-ready care strategies and share ideas with international colleagues. The role of GP practices and communities in promoting physical activity
- CityMove (City-based interventions to stimulate active Movement for health)
Physical activity promotion plays a crucial role in primary care, serving as a foundational strategy to prevent and manage a wide range of chronic diseases such as diabetes, hypertension, and cardiovascular conditions. Primary care providers are well-positioned to identify patients who may benefit from increased physical activity and to offer brief, targeted interventions that encourage lifestyle changes. By linking with communities through local exercise programs, fitness centers, walking groups, or social prescribing initiativesāprimary care can extend its impact beyond the clinic. This collaborative approach can support patients in maintaining physical activity routines, addresses social determinants of health, and fosters a more integrated, community-oriented model of care that promotes long-term health and well-being. This session will share experiences from Belgium, Slovenia, and the Netherlands on how primary care providers and communities are promoting physical activity.

ÄujeÄnost v primarnem zdravstvu: Pot do manj izgorelosti in boljÅ”e oskrbe
- Katedra za družinsko medicino, Medicinska fakulteta, Univerza v Ljubljani
Zdravstveni delavci v primarnem zdravstvu se vsakodnevno sooÄajo z visoko stopnjo stresa, kar poveÄuje tveganje za izgorelost in vpliva na kakovost obravnave pacientov. ÄujeÄnost je dokazano uÄinkovita metoda za zmanjÅ”anje stresa, krepitev Äustvene stabilnosti ter izboljÅ”anje empatije in komunikacije. Na delavnici bomo predstavili znanstvene osnove ÄujeÄnosti ter preizkusili krajÅ”e vaje, primerne za izvajanje med delovnim dnem. Udeleženci bodo spoznali tudi možnosti za vkljuÄevanje ÄujeÄnosti v svojo kliniÄno prakso in okolje. Delavnica bo kombinacija kratkih predavanj, praktiÄnih vaj in skupinske razprave. Predhodne izkuÅ”nje niso potrebne.
